1. Acceptance of Terms
By accessing or using the My PA Resource website, platform, or any services offered by Elevated
Transformations, LLC d/b/a My PA Resource (“MPAR,” “we,” “us,” or “our”), you (“you” or “User”) agree to
be bound by these Terms of Service (“Terms”). If you do not agree, do not use this website or enroll in any
program.
These Terms constitute a legally binding contract under Florida law and the federal Electronic Signatures
in Global and National Commerce Act (E-SIGN Act, 15 U.S.C. §§ 7001 et seq.) and the Florida Electronic
Signature Act (Fla. Stat. § 668.50). Completing online enrollment constitutes your electronic signature and
your agreement to these Terms, the Privacy Policy, and any applicable program agreement.
2. Who We Are
My PA Resource is the registered fictitious name of Elevated Transformations, LLC, a Florida limited liability
company. We are an educational consulting company providing application strategy, coaching, mentorship,
personal statement guidance, interview preparation, and related resources to individuals preparing to apply
to physician associate (PA) programs.
My PA Resource is not a medical practice, healthcare provider, licensed admissions counselor, physician
associate program, or academic institution. Our founder’s PA-C credential is provided for biographical
context only and does not indicate that clinical or medical services are provided through this platform. See
our Medical Disclaimer for more information.
3. Eligibility
You must be at least 18 years of age to use this website or enroll in any MPAR program. By using this
website, you represent that you are at least 18 years old and have the legal capacity to enter into binding
contracts. MPAR does not knowingly collect personal information from individuals under 13. If you are under
13, do not use this website.
4. Enrollment and Program Agreements
Enrollment in any MPAR program is subject to the My PA Resource Master Services Agreement (“MSA”),
which is incorporated into these Terms by reference and governs the relationship between MPAR and
enrolled students. By completing online enrollment and checking the consent box at checkout, you agree
to the MSA in addition to these Terms.
The MSA contains important provisions including limitations on MPAR’s liability, a binding arbitration clause,
and a class action waiver. Please read it before enrolling. A link to the MSA is provided at checkout.
5. Intellectual Property
All content on this website and within MPAR’s programs, including written materials, templates, frameworks,
video content, recorded sessions, course curriculum, brand elements, logos, and all other materials, is the
exclusive property of Elevated Transformations, LLC d/b/a My PA Resource and is protected by U.S.
copyright, trademark, and other intellectual property laws.
You are granted a limited, personal, non-transferable license to access and use MPAR’s content solely for
your own non-commercial use in connection with your personal PA program application. You may not
reproduce, distribute, sell, create derivative works from, or share any MPAR content with any third party
without MPAR’s prior written consent. Unauthorized use may result in termination of your account and civil
or criminal liability.
6. Permitted and Prohibited Website Use
You agree to use this website only for lawful purposes and in accordance with these Terms. You agree
not to:• Access, scrape, copy, or reproduce any website content for commercial purposes
• Attempt to gain unauthorized access to any portion of the website or MPAR’s systems
• Upload or transmit viruses, malware, or any other harmful code
• Impersonate MPAR, its staff, or any other person or entity
• Use the website in any manner that could disable, overburden, or impair its functionality
• Post, transmit, or distribute any unlawful, defamatory, harassing, or fraudulent content
• Use automated tools or bots to interact with the website without MPAR’s written consent

8. No Guarantee of Outcomes
MPAR makes no representation, warranty, or guarantee that use of this website or enrollment in any MPAR
program will result in admission to any PA program, employment as a physician associate, or any specific
academic or professional outcome. All educational content is advisory and informational only. See our

9. Disclaimer of Warranties
THIS WEBSITE AND ALL CONTENT ARE PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS
WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO
WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, ACCURACY, OR
COMPLETENESS. MPAR DOES NOT WARRANT THAT THIS WEBSITE WILL BE UNINTERRUPTED,

10. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, MPAR, ITS MEMBERS, MANAGERS,
OFFICERS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL,
SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR
USE OF THIS WEBSITE OR ITS CONTENT, INCLUDING LOST PROFITS, LOST DATA, OR LOSS OF
EDUCATIONAL OPPORTUNITY, EVEN IF MPAR HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH
DAMAGES. MPAR’S MAXIMUM AGGREGATE LIABILITY ARISING FROM YOUR USE OF THIS

11. Indemnification

employees, and agents from and against any claims, damages, liabilities, costs, and expenses (including
reasonable attorneys’ fees) arising from: (a) your use of this website; (b) your violation of these Terms; (c)
your violation of any applicable law; or (d) any content you submit to this website or to MPAR.
12. Dispute Resolution
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-laws rules. Any
dispute arising from your use of this website that is not resolved informally shall be subject to binding
arbitration in Hillsborough County, Florida, under the American Arbitration Association’s rules, except that
either party may seek emergency injunctive relief from a court of competent jurisdiction. Enrolled students
are also subject to the dispute resolution provisions of the MSA, which govern the enrollment relationship.
To the extent not subject to arbitration, you consent to the exclusive jurisdiction and venue of the state and
federal courts in Hillsborough County, Florida.
13. ModificationsMPAR reserves the right to modify these Terms at any time. When changes are made, MPAR will update
the Effective Date at the top of this page. For enrolled students, MPAR will provide written notice of material
changes to the Terms before they take effect. Your continued use of this website after a modification
constitutes acceptance of the updated Terms. If you do not agree to the updated Terms, you must stop
using this website.

My PA Resource is committed to protecting your privacy. This Privacy Policy explains what personal
information we collect, how we use and protect it, with whom we share it, and what rights you have regarding
your data. By using our website or enrolling in any MPAR program, you consent to the data practices

This Privacy Policy is incorporated into and made part of MPAR’s Terms of Service and Master Services
Agreement. MPAR operates as a data controller with respect to the personal data of its users and enrolled
students.

• Identifiers: full legal name, email address, phone number, mailing address
• Account and enrollment data: username, password, program enrolled, enrollment date
• Payment data: billing name and address (note: raw card numbers are handled by our third-party
payment processor and are not stored by MPAR)
• Application materials: personal statement drafts, activity descriptions, academic history, and other
content you voluntarily submit for review
• Communications: emails, messages, and other correspondence you send to MPAR
• Community content: posts, comments, or other content you submit to the MPAR Online
Community
B. Information Collected Automatically:
• Device and browser data: IP address, browser type and version, operating system, device
identifiers
• Usage data: pages visited, time spent on pages, links clicked, referring URLs
• Cookies and tracking technologies: see Section 7 below
C. Information from Third Parties:
• Payment processors: confirmation of payment status (not raw card data)
• Scheduling platforms: appointment booking data
• Analytics providers: aggregated usage data
3. How We Use Your Information

4. How We Share Your Information
MPAR does not sell your personal information to third parties. We share personal information only in the
following circumstances:
A. Service Providers:
We share personal data with trusted third-party vendors who assist us in operating our business. These
include:
• Payment processors: e.g., Stripe, Square: processes payment transactions; does not receive
application content
• Scheduling platforms: e.g., Acuity, Calendly: manages session booking; receives name and email.
• Email platforms: e.g., Mailchimp, ConvertKit: delivers communications; receives name and email.
• Video platform: e.g., Zoom: hosts live sessions; subject to that platform’s terms.
• Community platform: e.g., Circle, Kajabi: hosts the MPAR Online Community; receives name,
email, and community posts.
• Website analytics: e.g., Google Analytics: collects anonymized usage data; see Section 7.
All service providers are contractually required to use personal data only for the specific services they
provide to MPAR and are prohibited from using it for their own marketing purposes.
B. Legal Requirements:
We may disclose your personal information if required to do so by applicable law, court order, subpoena,
or governmental authority, or if we believe in good faith that disclosure is necessary to protect the rights,
property, or safety of MPAR, its users, or the public.
C. Business Transfers:
If MPAR is involved in a merger, acquisition, or sale of all or substantially all of its assets, your personal
information may be transferred as part of that transaction. We will notify enrolled students of any such
transfer and of any choices they may have regarding their data.
D. With Your Consent:
We may share your information with third parties in other circumstances with your explicit prior consent.

MPAR retains your personal information for as long as necessary to fulfill the purposes for which it was
collected, including: the duration of your active enrollment; the period required to comply with applicable
legal obligations; the period necessary to resolve disputes or enforce our agreements; and any additional
period required for legitimate business purposes. Payment records are retained for a minimum of

Wellness Professionals Are Independent Third Parties
All Wellness Professionals made available through or referenced by My PA Resource are independent
contractors or third-party service providers. They are not employees, agents, partners, affiliates, or
representatives of Elevated Transformations, LLC d/b/a My PA Resource. MPAR does not employ,
supervise, direct, train, credential-verify, or control any Wellness Professional.Separate Engagement — No MPAR Involvement
Any services you receive from a Wellness Professional are provided under a separate, independent
arrangement between you and that Wellness Professional. My PA Resource is not a party to that
arrangement. MPAR has no obligations, liabilities, or responsibilities of any kind arising from or related to
your engagement with a Wellness Professional, including no responsibility for the quality, safety, accuracy,
legality, or outcome of any services provided.
Any contract, payment, liability, or dispute related to a Wellness Professional’s services is exclusively
between you and that professional.
No Endorsement or Warranty
A referral from My PA Resource does not constitute an endorsement, recommendation, warranty, or
guarantee of any Wellness Professional’s competence, professional credentials, licensure, scope of
practice, insurance coverage, or the quality or safety of their services. MPAR does not independently verify
the credentials or licensure of Wellness Professionals. You are solely responsible for investigating and

Welcome to the My PA Resource Community
The My PA Resource Online Community (the “Community”) is a private space for enrolled MPAR students
to connect, support one another, ask questions, and share their PA application journey. Access to the
Community is a privilege extended to eligible enrolled students, not a guaranteed right, and is subject to
these Community Rules at all times.By accessing the Community, you agree to these Rules in addition to MPAR’s Terms of Service and your
program agreement (the Master Services Agreement). Violation of these Rules may result in suspension
or permanent removal from the Community.
Purpose of the Community
The Community exists to support your PA school application journey in a positive, respectful, and
constructive environment. It is not a medical advice forum, a clinical consultation space, or a general social
media platform. Please use it accordingly.
Community Standards — What We Expect
All Community members are expected to engage with honesty, respect, and good faith. You agree to:
• Treat all members, MPAR coaches, and staff with basic professionalism and courtesy
• Share your genuine experience and perspective in a way that is helpful and accurate
• Respect the privacy and confidentiality of other members’ personal information and application
materials
• Use the Community for its intended purpose — supporting PA school applicants

Prohibited Conduct
The following conduct is strictly prohibited in the Community and may result in immediate removal:

private messages outside the Community without that member’s express written consent
• Reproducing, screenshotting, or sharing any MPAR content, Educational Materials, coaching
materials, or proprietary frameworks outside the Community platform, in whole or in part
• Soliciting, advertising, or promoting any external product, service, competing program, or
commercial offering without MPAR’s prior written approval
• Impersonating MPAR staff, coaches, or other members
• Posting false, misleading, or fabricated content of any kind
• Providing or soliciting medical advice, clinical guidance, diagnosis, treatment recommendations,
or other healthcare guidance
• Posting content that promotes or facilitates any illegal activity
• Engaging in any conduct that disrupts the Community, undermines MPAR’s educational mission,
or degrades the experience of other members
No Medical Advice in the Community
The Community is not a healthcare forum. No member, and no MPAR coach or representative, is authorized
to provide medical advice, clinical recommendations, or health-related guidance of any kind through the
Community. If a health-related question arises, members should be directed to consult a licensed
healthcare provider. MPAR coaches will not answer clinical or medical questions in the Community.
Content You Post
You are solely responsible for any content you post in the Community. By posting content, you represent
that you have the right to share it and that it does not violate any third party’s rights. You grant My PA
Resource a limited, non-exclusive, royalty-free license to display and retain your Community posts within
the Community platform for the purposes of operating the Community. MPAR will not publicly publish or
commercially use your Community posts outside the platform without your separate written consent.
MPAR does not endorse the views, recommendations, or advice of any individual Community member.
Member content is not reviewed or approved by MPAR before posting and does not represent MPAR’s
position or advice.MPAR’s Right to Moderate
MPAR reserves the right, but not the obligation, to monitor, review, edit, remove, or restrict any Community
content or member access at any time and in MPAR’s sole discretion, with or without advance notice.
Community moderation decisions are final.
MPAR is not liable for the conduct, statements, posts, or recommendations of any Community member.
MPAR is not responsible for any harm you may suffer as a result of relying on another member’s post,
recommendation, or experience.
Consequences of Rule Violations
Violations of these Community Rules may result in, at MPAR’s discretion: a warning; temporary suspension
from Community access; or permanent removal from the Community. Removal from the Community for a
Rule violation does not entitle you to a refund of any fees paid. Serious violations, including harassment,
threats, misappropriation of MPAR content, or sharing another member’s private information, may also
result in termination of your enrollment agreement and potential legal action.
Governing Terms
These Community Rules are incorporated into and governed by MPAR’s Terms of Service and Master
Services Agreement. In the event of a conflict, the Master Services Agreement controls. All disputes are
subject to the dispute resolution provisions of your program agreement.
